This case may occur when your Ubuntu OS installed from Usb iso installer. After using USB iso installer of Ubuntu 14/15, USB and Harddisk will mounted at the same time. Usually on installation process, GRUB will installed on the first Drive.

Type in command line:
# fdisk -l

For changing the grub that has been installed on USB (/dev/sda), move to master boot of hard disk (/dev/sdb), just type:
# grub-install /dev/sdb

Restart Ubuntu server, and try to unplug USB Flash Disk from your computer.

Note:
  1. The result of this tutorial at your own risk, the risk is your ubuntu server won't boot if GRUB not installed properly.
  2. When Installation of GRUB finished, be sure there are no errors reported.
